In consideration of the services of USMMA, LLC, their agents, volunteers, participants, employees, owners and all entities acting on their behalf, collectively referred to as USMMA, LLC hereafter, I hereby agree to release, waive, discharge, and hold harmless USMMA, LLC on behalf of myself, my children, my parents, heirs, assigns, and estate as follows:

A. I understand and acknowledge that the study and training of Yoga, Pilates, Martial Arts like Judo, Jiu-Jitsu, Kickboxing, Muay Thai, and self-defense entail known and unanticipated risks that could result in physical or emotional injury, paralysis, death, or damage to myself, to property, or to third parties. I understand that such risks simply cannot be eliminated, and some of these risks include, but are not limited to, heart attack, bruises, broken bones, or serious injury.

 B. I agree and promise to accept and assume all risks existing in this activity. My participation is completely voluntary, and I want to participate despite the known and unknown risks.

 C. I recognize that participation includes body contact and grabbing or manipulation of various body parts.

 D. I hereby release, waive, discharge, and agree to indemnify and hold harmless USMMA, LLC from any and all claims, demands, or causes of action, which are in any way connected to my participation in this activity or use of equipment or facilities, including any claims or causes of action arising from the negligent acts or omissions of USMMA, LLC, its agents, employees, volunteers, or entities acting on their behalf.

 E. Should USMMA, LLC or anyone acting on their behalf be required to incur attorney fees and costs to enforce this agreement, I agree to indemnify and hold them harmless for all such fees and costs.

 F. I certify that I have adequate insurance to cover any injury or damage I may cause or suffer while participating or else I agree to bear the costs of such problems. I further certify that I have no medical condition that could interfere with my safety or participation.

 G. I acknowledge that by signing this document, I waive my right to maintain a lawsuit against USMMA, LLC and its employees, agents, volunteers, participants, or entities on the basis of any claim from which I have released them herein.

 H. I hereby affirm that I have read and fully understand this document and that I have had the opportunity to ask any questions regarding its contents. I acknowledge that I have been given sufficient time to review and seek advice as to the terms and conditions of this agreement and voluntarily and knowingly agree to be bound by its terms. In addition, I hereby consent to the use of my image and likeness in photographs and videos taken by or on behalf of USMMA, LLC during my participation in these activities. I understand that these photos and videos may be used for promotional, marketing, and other purposes by USMMA, LLC without compensation to me. I acknowledge that USMMA, LLC owns all rights to these photos and videos, and I waive any right to inspect or approve the finished product. I release USMMA, LLC and its agents, employees, and all entities acting on their behalf from any claims or liability that may arise from the use of my image and likeness in these photos and videos. By signing below, I acknowledge that I have read and fully understand this document and its provisions regarding the use of photos and videos, and I consent to the use of my image and likeness as described above.
